New Patient Information

Please take a moment to familiarize yourself with our office policies and procedures.

Payment Policy ~ Payment is required at the time of service unless other arrangements have been previously made. For your convenience, we accept cash, personal checks, MasterCard and Visa debit/credit cards.

Insurance Billing ~ We are currently not contracted with ANY insurance companies/plans, and are considered an OUT OF NETWORK PROVIDER; We encourage patients to call their insurance company for their specific reimbursements. We will be happy to provide you with a “coded bill”, which YOU can submit to your insurance provider.

Missed Appointments ~ We charge $50.00 for appointments missed or cancelled in less than a 24 hour notice (business hours). If you need to reschedule your appointment, please give us 24 business hours’ notice. The office is closed on Mondays, so appointments scheduled for Tuesday must be cancelled by the end of business at 4pm on Friday.

New Patients ~ New patient appointments are approximately 2 hours in duration for a $214.00 fee, and we ask for a $100.00 deposit at the time of scheduling to cover the missed appointment fee of $50 per hour, which is applied to your appointment and/or fully refundable if cancelled at least 48 business hours in advance. We ask for this deposit due to the extended appointment time and in consideration of our other patients.

New patient appointments include a full physical exam, lab review, history and comprehensive intake from a western and eastern medicine perspective ~ we ask that you come a little early to fill out paperwork or download the intake forms and fill them out prior to visit.

An average treatment plan for chronic conditions is once weekly for the first 4-6 weeks. Follow-up visits are typically 45 minutes to 1 hour and cost $119.00. As the patient improves, visits are reduced to once every 10 days; 2 weeks; once per month; and, so on. Many patients choose to continue with care as desired to maintain their health.

***Please bring any supplements or prescription medications that you are currently taking to your first office visit. If you have any LABORATORY results from within 2 years from another facility, it would be helpful for you to bring those to the first visit as well. ***

Brief Office Call ~ lab results; disp/Rx refills; brief discussions and/or questions for Dr. ~ $81.00 ½ hr

First Office Call ~ $214.00 (see above) 90 min – 2 hours

Return Office Call ~ $119.00 - 45 min – 1 hour

We are happy to answer short questions and/or clarify instructions from a previous visit on the phone or via email without charge. If you have a question about a new topic, please make an appointment.
